  • Provide effective brief interventions to assist primary care patients in successfully achieving goals.
  • Provide screening assessment, diagnosis, and referral on an as needed basis.
  • Evaluate crisis situations and apply appropriate interventions.
  • Actively participate in meetings that support the integrated health care model to provide comprehensive care for patients.
  • Assist in the detection of "at risk" patients and develop plans to promote wellness.
  • Assist the integrated care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
  • Provide assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders.
  • Educate patients, families, and staff in the BHC's areas of expertise.
  • Actively participate in weekly clinical consultation meeting with the Behavioral Health Team
  • Keep patient records up to date, comprehensive in nature in accordance with federal, state, and board regulations.

As an Integrated Behavioral Health Consultant a typical day might include:

  • Apply solid working knowledge of biopsychosocial models of human behavior and development for assessment of patient care to include knowledge of the use of adult and pediatric screening instruments, as well as the ability to screen for and identify substance abuse problems.
  • Screen for mental health concerns, substance abuse concerns, adverse health behaviors (including their contribution to chronic medical illnesses), stress-related physical symptoms, ineffective patterns of health care utilization, and adjustment to new and chronic medical conditions. Apply first line interventions in a time-limited setting for a broad spectrum of physical and mental health problems experienced by patients and families throughout the lifespan as presenting in primary care (pediatrics, Internal Medicine, and Family medicine populations) or Specialty Care (ob/gyn, dermatology, cardiology, etc.)
  • Provide patient and family education effectively and sensitively to encourage compliance with medical and behavioral treatment plans. To include the skill and expertise to discuss common psychotropic medications with a patient, including indications, effects and side effects, and demonstrate the ability to appropriately recommend initiating medication to primary care physician.
  • Effectively utilize electronic health record and verbal communication to concisely communicate relevant information to medical providers in a timely manner. Ability to concurrently document during patient sessions. To include performance of various clinical, compliance, and administrative duties required to meet various billing and medical documentation requirements.
  • Effectively assess for and manage suicidal and homicidal risk, as well as concerns for child or elder abuse and neglect, using effective and evidence-based methods (PHQ-9, Columbia Risk Assessment). When applicable, collaborate and coordinate care with the Crisis team, contact police and/or Child Protective Services and communicate effectively with receiving emergency department.
  • Consult with other organizations to develop integrated programs.

Qualifications needed:

  • Master's Degree in Social Work (MSW) from an accredited school
  • Current License in the State of Idaho as a BH Provider
  • Ability to receive clearance through the National Practitioner Database
  • Working knowledge of the basic principles of health, illness, and wellness including current best Practice in effective delivery of preventive and primary behavioral health care and health education to low-income and special needs populations.
  • Proven clinical competence and outcome-effective experience providing primary and preventive health services.
  • Skill in behavioral health services primary care delivery and ability to establish and maintain effective working relationship with patients and staff.
  • Experience with rural community health care, public health principles and practices desired.
  • Strong written and interpersonal communication skills, creative thinker and pa proven collaborator.
